1) Complete address in which the show will be held ( This is very important, please do not leave out)
2) Date and time you are looking for
3) Length of show you would like to reserve ( 30 minutes, 45 minutes or 60 minutes)
4) Two phone numbers where you can be reached
5) Age of birthday child or age range of the group (for schools, daycares, churches, etc.)
6) How you found out about The Creature Teacher or came across this website (i.e. recommendation by a friend, name of Magazine or internet-- if internet please name the website that referred you to my website, ie. partypop.com or critterplaces.com)
7) Number of children expected to attend the program (i.e, less than 20, 50-100, 200 or more.) Keep in mind that infants and toddlers may not attend the show so please exclude them from your total calculations.
8) If your school, daycare, church etc, requires an invoice, I'll need the organization whose name should apprear on the invoice.
